Refer for an OT Driving Assessment
Use this form to refer a client for an OT Driving Assessment (OTDA) with Intracare Services. Our credentialled OT Driving Assessors work with new drivers, people returning to driving after injury or illness, and those requiring a fitness-to-drive evaluation. Please include as much clinical detail as possible to help us match the referral to the right assessor and pathway.
